When calling to schedule an appointment, please have the following information ready:

• Patient's full name
• Patient's date of birth
• Best phone number to reach patient (home, work, or cell phone)
• Social Security number
• Home address (city, state, and zip code)
• Diagnosis (if already made)
• Name of referring physician
• Name of primary insurance and policy number
• Name of secondary insurance and policy number (if applicable)
• Name of the insured party

Any previous test results that you can bring to the first visit are also helpful, including:

• Reports from care recently provided outside NYU Langone Medical Center and the NYU Clinical Cancer Center
• Prior chest X-rays, CAT scans, mammography reports and films, PET scans, MRI scans, and bone scans that are available
• Pathology slides and reports
• Results of other recent tests, such as EKGs, blood tests, etc.
• A list of questions you have about your diagnosis, treatment options, or other issues
