Instructions For Submitting A Protocol To NYUCI PRMC

To submit a protocol to NYUCI PRMC
1) Download the appropriate forms from below
2) Email the completed forms and your protocol (for initial submissions only) to prmc.coordinator@nyumc.org
3) Mail signed hard copies of the completed forms and your protocol to:

Form Downloads
For New Protocols, you will need to download and complete the following PRMC forms. THE SIGNED PROGRAMMATIC APPROVAL FORM MUST BE RECEIVED BY THE SUBMISSION DEADLINE IN ORDER FOR THE PROTOCOL TO BE PLACED ON THE AGENDA FOR THAT MONTH.
